For your convenience we accept Visa, MasterCard, American Express, and Discover; therefore payment is due at the time service is rendered unless other arrangements have been made in advance. As a courtesy to our patients, we file to your insurance all procedures covered under your plan. We will assist you in determining the best way to maximize your policy in order to meet your treatment needs. *Please note that your policy is a contract between You and your Insurance Provider.
If a claim is not paid within a timely manner, will we ask that you pay the balance due?
YES. We will provide you with any needed documentation in order for you to acquire a reimbursement from your Insurance Provider. Although we strive to make your visit as comfortable as possible, you are ultimately responsible for all treatment costs.
What do you need in order for us to determine your benefits?
While some companies provide a Dental Card, many do not. Do provide us with: the name and telephone number of your Insurance Provider, Group Name/Number, Member ID, and Identification (Drivers License).
